Nonprofit Eligibility Form – (for Nonprofit to Complete)

This form is to be completed by the Executive Director or Board Chair of a nonprofit to confirm eligibility for funding consideration. Once eligibility is verified, the nonprofit will be added to our list of pre-qualified nonprofits eligible for nomination and consideration for an Impact Donation.

This form only needs to be filled out one time per nonprofit and will remain on file.
